ASCO 2026: Five Cancer Studies That Redefine What GLP-1s Can Do

The 2026 ASCO Annual Meeting produced a cluster of five real-world studies that are reshaping how oncology teams think about GLP-1 receptor agonists. Here's what each study found, who conducted it, and what it means.

Study 1: GLP-1 Use and Cancer Progression

Led by Mark David Orland, MD, of Cleveland Clinic's Taussig Cancer Institute, this analysis examined GLP-1 receptor agonist use across a decade of patient records (2015-2025). The investigators found associations between GLP-1 use and either reduced cancer incidence or improved cancer outcomes across multiple tumor types.

Orland characterized the results as "exploratory rather than conclusive," noting that cancer therapies evolved substantially during the study period. The dataset's strength is its size; its limitation is the observational design.

Study 2: Breast Cancer Prevention in High-Risk Women

A second study examined whether GLP-1 medications could serve as primary prevention for breast cancer in high-risk women. The biological rationale: obesity and chronic inflammation are established breast cancer risk factors, and GLP-1 drugs address both. The study found encouraging results, though the investigators emphasized the need for prospective clinical trials.

Study 3: GLP-1 and Immunotherapy Response

A propensity-matched real-world analysis compared GLP-1 receptor agonists head-to-head with other diabetes medications in patients receiving immune checkpoint inhibitors. The analysis found improved survival outcomes in the GLP-1 group, suggesting potential synergy between GLP-1 medications and cancer immunotherapy.

Study 4: Metastatic Breast Cancer and CDK4/6 Inhibitors

This analysis found that GLP-1 use was associated with a 30% reduction in mortality risk in patients with hormone receptor-positive, HER2-negative metastatic breast cancer who were receiving CDK4/6 inhibitor-based regimens.

Study 5: GLP-1 and Metastatic Progression Across Tumor Types

The fifth study examined GLP-1 use and metastatic progression across multiple cancer types, finding broadly favorable signals.

The pattern: Five independent research teams, using different methodologies, studying different cancer types, all found positive signals. While no individual study is conclusive, the consistency of the direction is noteworthy and warrants controlled clinical trials.

Limitations to Acknowledge

All five studies were observational or retrospective — they show correlation, not causation. Patients taking GLP-1 medications may differ from non-users in ways that independently affect cancer outcomes (healthier behaviors, better healthcare access, more regular monitoring). Prospective randomized trials are needed to establish whether GLP-1 medications truly prevent or slow cancer.
