Novo Nordisk vs. Eli Lilly: The $200 Billion GLP-1 Market War in Charts
The GLP-1 market generated approximately $50 billion in revenue in 2024. Goldman Sachs projects it will exceed $200 billion by 2030. Two companies control virtually all of it: Novo Nordisk (Ozempic, Wegovy, Rybelsus) and Eli Lilly (Mounjaro, Zepbound, Foundayo). Where Novo Leads
Clinical evidence depth. Semaglutide has the largest clinical trial program in obesity history — STEP 1–5, SELECT, FLOW, STEP-HFpEF, OASIS 1–4, and numerous substudies. The SELECT trial's 20% reduction in major cardiovascular events gives Wegovy a cardiovascular indication that Zepbound does not have. For insurers and prescribers who prioritize hard outcomes data, this matters.4
Installed base. Wegovy has been prescribed to approximately 3.2 million people in the US. Ozempic's diabetes user base is even larger. Brand recognition and prescriber familiarity are substantial competitive advantages.
Where Lilly Leads
Head-to-head efficacy. SURMOUNT-5 settled the debate: tirzepatide produces 20.2% weight loss versus semaglutide's 13.7% — a 47% greater relative reduction. For patients and prescribers who prioritize maximum weight loss, Zepbound wins on the numbers.4
Oral convenience. Foundayo is the first small-molecule oral GLP-1 — no fasting, no water restrictions, any time of day. Novo's Wegovy pill is effective but requires a 30-minute fasting protocol. For adherence-sensitive patients, Foundayo has a genuine practical advantage.5
Pipeline breadth. Retatrutide (GLP-1/GIP/glucagon triple agonist) produced 24.2% weight loss in Phase 2 — potentially the most effective obesity drug ever tested. If Phase 3 confirms this, Lilly will have both the best injectable and the best oral on the market.
The Direct-to-Consumer War
Both companies are building vertically integrated pharmacy channels to bypass PBMs and reach patients directly:
- NovoCare: Novo Nordisk's direct pharmacy for Wegovy and Ozempic. Self-pay pricing, savings cards, delivery.
- LillyDirect: Eli Lilly's direct channel for Zepbound, Mounjaro, and Foundayo. $149/mo self-pay for Foundayo, coordinated delivery.6,7
