The Oral GLP-1 Era Has Arrived: A Research-Based Timeline
The transition from injectable-only GLP-1 therapy to a full menu of oral options happened faster than most industry observers expected. Here's the research-based timeline of how we got here.
The Timeline
| Date | Milestone | Significance |
|---|---|---|
| Sep 2019 | Rybelsus (oral semaglutide) FDA-approved for T2DM | First oral GLP-1. Required SNAC absorption enhancer. Moderate bioavailability. |
| 2020-2023 | Multiple oral GLP-1 candidates enter clinical trials | Small-molecule approaches bypass the peptide absorption challenge. |
| Dec 2025 | Oral Wegovy FDA-approved for weight loss | First oral GLP-1 specifically indicated for obesity. |
| Jan 2026 | Oral Wegovy commercial launch | Available at pharmacies. No cold-chain shipping needed. |
| Apr 2026 | Foundayo (orforglipron) FDA-approved | First non-peptide small-molecule GLP-1. Different mechanism than semaglutide. |
| May 2026 | Ozempic tablets launched at 70,000+ pharmacies | Oral semaglutide with cardiovascular indication. $149/mo self-pay. |
| Jun 2026 | Elecoglipron enters Phase III | AstraZeneca's oral small-molecule GLP-1. Third distinct oral approach. |
| 2027-2028 | Additional oral candidates expected | Pfizer, Amgen, and smaller biotechs developing oral GLP-1s. |
Why Oral Matters
Oral administration removes the primary patient barrier (needle aversion), eliminates cold-chain shipping requirements, simplifies travel, and — for small-molecule drugs — dramatically reduces manufacturing costs. The shift to oral formats is expected to expand the total addressable patient population significantly, bringing GLP-1 therapy to millions who avoided injectable treatment.
