Wegovy Pill Prescribing Information: The Dosing Rules That Matter

Updated October 20268 min readSources cited inline

The Wegovy pill (oral semaglutide 25 mg) works only if it is taken a specific way. The label says one tablet each morning on an empty stomach, with no more than 4 ounces of plain water, then at least 30 minutes before any food, drink, or other oral medicine. The dose climbs from 1.5 mg to 25 mg over 90 days, and the label offers no lower pill maintenance dose. If 25 mg can't be tolerated, the label points to the injection instead.

Key takeaways

  • The administration rules (empty stomach, 4 ounces of water at most, 30-minute wait) are in the label itself, not just patient tips.
  • The escalation is fixed: 1.5 mg, 4 mg, 9 mg, then 25 mg daily from day 91.
  • A missed dose is skipped, not made up later in the day.
  • Rybelsus uses the same administration rules but is a type 2 diabetes drug with a 9 mg or 14 mg ceiling. The two should not be combined.
  • The pill raises levothyroxine exposure by about 33%, which matters for anyone on thyroid medicine.

What the Wegovy pill is approved for

Novo Nordisk announced FDA approval of the Wegovy pill on December 22, 2025, calling it the first oral GLP-1 approved for weight management. It went on sale in the US on January 5, 2026, according to Reuters.

The current FDA label, revised June 2026, gives the tablets two indications, each alongside a reduced-calorie diet and more physical activity:

The Wegovy injection also carries a MASH (fatty liver disease) indication; the tablets do not. The label also notes that the tablets have not been studied for weight reduction in adults with type 2 diabetes.

The administration rules, word for word

The label states the core instruction in one sentence:

Take one WEGOVY tablet orally once daily on an empty stomach in the morning with water (up to 4 ounces).

The rest of section 2 of the prescribing information fills in the details:

  1. Empty stomach, in the morning. The tablet is the first thing taken that day.
  2. Plain water only, up to 4 ounces. No coffee, juice, or other liquids with the tablet. Four ounces is half a cup.
  3. Swallow it whole. Do not split, crush, chew, or dissolve it.
  4. Wait at least 30 minutes before eating, drinking anything, or taking other oral medicines.
  5. One tablet per day, maximum.

The rules exist because semaglutide is a peptide, and the gut absorbs peptides poorly. The empty stomach, small sip of water, and waiting period are the conditions under which the tablet delivers enough drug. That is why the label frames them as dosing instructions rather than suggestions.

Practical note: One simple routine is to keep the tablet and a measured half-cup of water by the bed and take it on waking. Anyone who takes other morning medicines should ask a pharmacist how to schedule them around the 30-minute window.

The dose steps

The label's escalation table moves every 30 days and ends at a single maintenance dose.

Two details stand out. First, if a dose step is not tolerated, the label says to consider delaying the next step rather than dropping back. Second, if 25 mg is not tolerated, the label says to consider switching to the Wegovy injection at 1.7 mg once weekly. The label does not describe staying on 4 mg or 9 mg tablets long term for weight management.

All four tablets are white to light yellow and debossed with "novo" and the dose number. The 1.5, 4, and 9 mg tablets are round, and the 25 mg tablet is oval.

Missed doses

The instruction is to "skip the missed dose and take the next dose the following day." Taking it later in the day would usually mean taking it on a non-empty stomach, so the label doesn't allow a catch-up dose.

Switching between the pill and the injection

The label gives exact switching instructions in both directions:

The label lists no switching path from injection doses below 2.4 mg, so those switches are left to the prescriber.

How the Wegovy pill differs from Rybelsus

Rybelsus is also oral semaglutide, and the administration rules are nearly identical. The Rybelsus label (revised October 2025) specifies an empty stomach, up to 4 ounces of water, and a 30-minute wait. The products differ in what they treat and how high the dose goes.

The overlap in strengths can cause confusion. A "4 mg semaglutide tablet" could be a Wegovy escalation dose or a Rybelsus R2 maintenance dose. The Wegovy label says not to use it with other semaglutide products or other GLP-1 drugs, and the Rybelsus label warns that its two formulations are not substitutable milligram for milligram and should not be used at the same time. Always check the brand name on the bottle, not just the number.

The Rybelsus label also shows that oral semaglutide doses don't translate simply. For switching between its own two formulations, it pairs R1 7 mg with R2 4 mg and R1 14 mg with R2 9 mg, and it says the formulations are "not substitutable on a mg per mg basis." Doses only make sense within one product.

Interactions and safety points in the label

The boxed warning is the same as for the injection: semaglutide caused thyroid C-cell tumors in rodents, and the drug is contraindicated with a personal or family history of medullary thyroid carcinoma or MEN 2. The most common side effects are gastrointestinal. The label's list of reactions reported in at least 5% of patients also includes dysesthesia (unusual skin sensations), headache, fatigue, dizziness, and hair loss.

How it compares on rules with Foundayo

The other oral GLP-1 for weight, Lilly's Foundayo (orforglipron), has no food or water rules. Its label says to take it once daily "with or without food." For someone who reliably takes a pill on waking, the Wegovy rules are a small routine. For someone with a busy medication schedule, they are a real constraint worth discussing with a prescriber.

What it costs

At launch, Reuters reported self-pay prices of $149 a month for 1.5 mg and $299 for 9 mg and 25 mg, with the 4 mg price scheduled to rise. Novo's April 2026 NovoCare price guide lists self-pay pill prices of $149, $199, and $299 a month by dose, and an insurance savings offer as low as $25 a month (up to $100 in monthly savings, government-insured patients excluded). Check the current guide before filling, since these offers change.

Bottom line on safety: The 30-minute wait and 4-ounce limit are part of how the drug is dosed. If your routine makes them hard to follow, raise it with your prescriber rather than adjusting on your own.

Frequently asked questions

How much water can I take the Wegovy pill with?

The label says up to 4 ounces of plain water, on an empty stomach in the morning. Other liquids should not be used to take the tablet.

How long do I wait after the Wegovy pill before eating?

At least 30 minutes before eating, drinking anything, or taking other oral medicines, according to the label.

What if I miss a dose of the Wegovy pill?

The label says to skip the missed dose and take the next dose the following day, rather than taking it later the same day.

Is the Wegovy pill the same as Rybelsus?

Both are oral semaglutide with the same water and wait rules, but Rybelsus is approved for type 2 diabetes and tops out at 14 mg (R1) or 9 mg (R2). The Wegovy pill is for weight management and cardiovascular risk reduction and is maintained at 25 mg. They should not be used together.

Can I stay on a lower Wegovy pill dose?

The label's schedule ends at 25 mg daily. If 25 mg is not tolerated, it says to consider switching to the Wegovy injection at 1.7 mg weekly, so discuss options with your prescriber.
