The Wall Street Journal documented dreams of "cabinet shopping at Home Depot with Clint Eastwood" and "preparing to rob a museum with Jennifer Lopez and Ben Affleck." Users describe being able to remember their dreams in startling detail—something many say they couldn't do before starting the medication.
What Does Novo Nordisk Say?
"'Abnormal dreams' was not a commonly reported adverse event during the clinical development programs of Ozempic or Wegovy, and therefore is not listed as a potential side effect in the product prescribing information of these two medicines."— Allison Schneider, Director of Media Relations, Novo Nordisk
However, Novo Nordisk has acknowledged receiving post-marketing reports:
"In later reports received by Novo Nordisk in 2022, 'abnormal dreams' were reported. However, since these adverse events are reported voluntarily from a population of uncertain size, it is not possible to reliably estimate their frequency... it is not possible to establish a cause-effect relationship between the products and the adverse event."
Why Might This Happen?
While research is limited, scientists have proposed several mechanisms:

- GLP-1 receptors in the brain: GLP-1 receptors exist in reward-related brain areas. The gut hormone these drugs mimic has direct effects on brain activity.
- Dopamine system overlap: "There is overlap of GLP-1 receptors with dopamine receptors, and there is likely some hormonal effect the GLP-1 medications have on mood and dreams," says Dr. Inessa Fishman, MD.
- Neurotransmitter modulation: According to Kim et al. (2020), GLP-1 modulation affects serotonin, dopamine, GABA, and glutamate release. Altered levels of serotonin and dopamine can cause vivid or bizarre dreams.
- REM sleep changes: Some research suggests semaglutide may influence REM sleep, the stage where most vivid dreams occur.
- Blood sugar stabilization: More stable blood sugar overnight may alter sleep architecture, potentially affecting dream recall.

Is This Related to Other Brain Effects?
The dream phenomenon fits a pattern of GLP-1 drugs affecting the brain in unexpected ways. Users also report:

What About Sleep Problems?
Some users report insomnia or disrupted sleep, though this is also not listed as an official side effect. Potential causes include:
- GI side effects: Nausea, reflux, or discomfort can disrupt sleep
- Eating timing: Eating less or eating differently may affect sleep patterns
- Blood sugar changes: More stable glucose can change sleep architecture
- Vivid dreams themselves: Intense dreams may cause waking or lighter sleep
Clinical trials for Wegovy found insomnia rates were roughly the same in semaglutide groups versus placebo groups, suggesting the drug itself may not directly cause sleep problems.
Note: Tirzepatide May Be Different
Interestingly, vivid dreams have been reported more commonly with semaglutide (Ozempic/Wegovy) than with tirzepatide (Mounjaro/Zepbound). GoodRx notes that "nightmares and vivid dreams haven't been reported as side effects of Trulicity [dulaglutide]." Different GLP-1 agonists may have different brain effects.
What Can You Do About It?
If you're experiencing disruptive vivid dreams:
