$50 GLP-1s for Medicare Patients Start July 1 — Here's Exactly Who Qualifies and What's Covered
CMS's Medicare GLP-1 Bridge goes live July 1, 2026. Foundayo, Wegovy, and Zepbound KwikPen at $50/month for eligible Part D beneficiaries. Here's everything you need to know — including who's excluded.
What the Medicare GLP-1 Bridge Actually Is
On July 1, 2026, the Centers for Medicare & Medicaid Services (CMS) will launch one of the most consequential healthcare access programs in years: the Medicare GLP-1 Bridge, a short-term demonstration that provides eligible Medicare Part D beneficiaries with access to select GLP-1 medications for a flat cost of $50 per monthly supply.
The program runs from July 1, 2026 through December 31, 2027, and serves as a bridge to the larger BALANCE Model (Better Approaches to Lifestyle and Nutrition for Comprehensive hEalth) launching in Medicare Part D in January 2027.
Which Medications Are Covered
As of the April 6, 2026 update, three medications are eligible — but not all formulations:

If you currently use Zepbound single-dose vials or single-dose pens, you will not qualify through the Bridge. Only the KwikPen formulation is included. Talk to your provider about switching if you want Bridge coverage.

Additionally, prior authorization is required. CMS has not yet released the specific clinical criteria, but the medications must be used "to reduce excess body weight and maintain weight reduction." Details are expected before the July 1 launch.
How It Connects to the BALANCE Model
The Bridge is a short-term demonstration. The longer-term vision is BALANCE — a voluntary model where manufacturers, state Medicaid agencies, and Part D plan sponsors collaborate to cover GLP-1 medications paired with evidence-based lifestyle supports. BALANCE launches in Medicaid as early as May 2026 and in Medicare Part D in January 2027.
The Bridge essentially gives CMS time to collect utilization data and prepare Part D sponsors for broader coverage under BALANCE.
What About Compounded GLP-1s?
The Medicare GLP-1 Bridge covers only FDA-approved brand-name medications. Compounded semaglutide and tirzepatide are not included. And in a separate development, the FDA has proposed permanently excluding semaglutide, tirzepatide, and liraglutide from the 503B Bulks List — with a public comment period open through June 29, 2026. If finalized, this would end large-scale compounding of these drugs even in the event of future shortages.
Before July 1: Confirm your plan type with your Medicare carrier. Talk to your doctor about whether a GLP-1 is appropriate for you. If you're on compounded GLP-1s, start exploring the transition to brand-name formulations. CMS has said additional beneficiary information will be released as the program launches.
